Sleep problems can have many causes, including hormonal changes, chronic stress, pain, digestive problems, thyroid disorders, sleep apnea, and medication side effects. Sometimes several factors are present at the same time, which is why simply trying to “relax more” does not always solve the problem.
Difficulty falling asleep is only one form of disrupted sleep. Some people fall asleep easily but wake repeatedly during the night, wake too early, sleep lightly, or wake feeling as tired as they were before going to bed.
Understanding what may be driving the sleep problem is an important first step.
Sleep changes are common during perimenopause and menopause.
Changes in estrogen and progesterone can affect body temperature, mood, and sleep regulation. Hot flashes and night sweats can cause awakenings, but sleep can also become lighter or more fragmented even without obvious night sweats.
For some women, sleep problems begin years before their periods stop completely.
Sometimes the body is exhausted, but the nervous system is still highly activated.
You may feel physically tired while your mind keeps going. Your heart rate may feel elevated, your muscles may remain tense, or you may wake suddenly and find it difficult to settle again.
This is often described as feeling “wired but tired.”
Ongoing psychological stress can keep the body's stress-response systems activated. Cortisol normally follows a daily rhythm, with higher levels in the morning and lower levels at night. Chronic stress and disrupted sleep can interfere with this rhythm.
This does not mean that every person with insomnia has “high cortisol.” It means that stress-system dysregulation can be one contributor to poor sleep.

That does not always happen.
Prolonged physical or emotional demands can leave a person exhausted while the brain and nervous system remain alert. This can create the frustrating combination of needing sleep badly but being unable to achieve restorative sleep.
Poor sleep then adds another layer of stress the following day, creating a cycle that can be difficult to interrupt.
Both an overactive and an underactive thyroid can interfere with sleep.
An overactive thyroid can contribute to anxiety, increased heart rate, heat intolerance, and difficulty sleeping. An underactive thyroid is more commonly associated with fatigue, but it can also contribute to poor sleep quality.
If thyroid dysfunction is suspected, appropriate medical evaluation and laboratory testing are important.
Digestion and sleep can affect each other.
Acid reflux, bloating, abdominal discomfort, food reactions, or the need to use the bathroom can interrupt sleep directly.
Late meals, alcohol, caffeine, and certain foods can also affect sleep in susceptible people.
At the same time, ongoing poor sleep can influence digestion and appetite, creating another two-way cycle.
Pain is one of the most direct reasons people struggle to sleep.
Back pain, joint pain, headaches, pelvic pain, nerve pain, injuries, and other chronic discomfort can make it difficult to find a comfortable position or can repeatedly wake someone during the night.
Poor sleep can also increase sensitivity to pain the following day. This can create a cycle in which pain disrupts sleep and inadequate sleep makes pain harder to tolerate.
Sometimes a person appears to sleep for seven or eight hours but is repeatedly pulled out of deeper sleep because breathing is interrupted.

Suspected sleep apnea deserves medical evaluation because it is more than simply a sleep-quality problem.
Medications can also affect sleep.
Depending on the medication and the individual, a drug may cause insomnia, restlessness, daytime sleepiness, vivid dreams, or changes in normal sleep patterns.
If sleep changed after starting or changing a medication, discuss the timing with the prescribing healthcare professional rather than stopping the medication on your own.
When sleep problems occur alongside chronic stress, nervous system overactivation, exhaustion, or difficulty relaxing, quantum biofeedback can be used to look at the body's current stress and reactivity patterns.

Persistent sleep problems are worth investigating rather than assuming they are simply caused by stress.

Identifying the likely drivers provides a much clearer direction than treating every case of poor sleep in the same way.
